Sustainability and Transformation Partnership
What is an STP?
NHS England has asked every local health and care system to work together to produce a multi-year Sustainability and Transformation Plan (STP), showing how local services will evolve and become sustainable over the next five years and ultimately delivering the Five Year Forward View vision for the NHS.
Each local health and care system has to be able to answer the following questions:
- How will we close the health and wellbeing gap?
- How will we drive transformation to close the care and quality gap?
- How will we close the finance and efficiency gap?
